IV Therapy: What Is It and Is It Right for You?

Intravenous (IV) therapy has become increasingly popular, but it can be difficult to know what it actually involves—and whether it is something you need.


IV therapy delivers nutrients, vitamins, minerals, and other appropriate compounds directly into a vein. Because these substances are delivered intravenously, they enter the bloodstream without first passing through the digestive system.


But IV therapy isn't automatically better than taking something by mouth, and more isn't necessarily better.


In my practice, IV therapy is considered as one potential tool within an individualized naturopathic treatment plan. Whether it is appropriate depends on your health history, nutritional needs, medications and supplements, goals, and the evidence for the specific treatment being considered.

What is IV therapy?

IV therapy, short for intravenous therapy, involves administering a solution directly into a vein through a small catheter.


Depending on the clinical situation, an IV infusion may contain vitamins, minerals, amino acids, antioxidants, or other nutrients. The specific ingredients and doses should be selected based on the individual's clinical needs and treatment goals.


One of the main differences between intravenous and oral administration is how the substance enters the body. IV administration bypasses the gastrointestinal tract and can produce different concentrations in the bloodstream than oral supplementation.


This can be clinically useful in certain circumstances—but it does not mean that IV therapy is inherently more effective for everyone.


Why might someone consider IV therapy?

There are a number of reasons why intravenous nutrient therapy may be considered. The goal isn't simply to provide a nutrient, but to determine whether IV administration offers a meaningful advantage over taking that nutrient orally.


Depending on the nutrient, your health history, and your individual circumstances, IV therapy may be considered when:

  • Oral absorption may be impaired. Conditions affecting the gastrointestinal tract—including certain gastrointestinal disorders, inflammatory bowel disease flares, previous removal of portions of the GI tract, or other factors affecting absorption—may make it more difficult to absorb some nutrients from oral supplements. In some individuals, age-related changes may also affect nutritional status and the ability to meet nutrient needs through oral intake alone.

  • Oral supplementation has not been sufficient. In some situations, a nutrient deficiency may persist despite an appropriate trial of oral supplementation. IV administration can provide an alternative route when oral supplementation isn't achieving the desired result. Iron is one example where the appropriate route of administration may depend on the severity of deficiency, response to oral iron, tolerability, and individual circumstances.

  • Higher doses or blood concentrations are desired. Some nutrients can reach substantially higher concentrations in the bloodstream when administered intravenously than when taken orally. High-dose vitamin C is one example where IV administration allows doses and blood concentrations that cannot be achieved through oral supplementation alone.

  • Oral administration changes how a nutrient is handled by the body. Some nutrients are substantially affected by digestion and first-pass metabolism in the liver when taken orally. In certain circumstances, intravenous administration may alter the nutrient's bioavailability and exposure.

  • Oral intake is difficult or insufficient. There may be circumstances where someone is unable to consistently meet their nutritional needs through food and oral supplementation, and an intravenous route may be considered as part of an individualized treatment plan.


Importantly, none of these situations automatically means that IV therapy is necessary or preferable. The appropriate route depends on the specific nutrient, the reason it is being used, the available evidence, and your individual health circumstances.


The question isn't simply "What can we put in an IV?" It's "Is there a good reason to give this nutrient intravenously?"


Is IV therapy better than oral supplements?

Not necessarily.


For many vitamins and nutrients, oral supplementation is effective, convenient, less invasive, and more appropriate.


IV administration may have advantages in specific clinical situations, but it also involves risks and requires a trained healthcare professional to administer the treatment.


For example, intravenous vitamin C can achieve substantially higher blood concentrations than oral vitamin C. This is one reason IV vitamin C is being studied in certain cancer settings. However, that does not mean high-dose IV vitamin C is appropriate for everyone, or that it should be considered a treatment for cancer.


The appropriate route depends on what we're trying to accomplish and what is appropriate for you.


What about IV therapy for cancer patients?

This is an area where extra caution is important.


People undergoing cancer treatment may be interested in IV therapy for a variety of reasons, including nutritional support or as part of an individualized supportive care plan.


However, IV nutrient therapy should not be used as a substitute for conventional cancer treatment.


If you're receiving chemotherapy, immunotherapy, targeted therapy, radiation, or another cancer treatment, any IV nutrient or supplement needs to be considered in the context of your treatment plan.


Some nutrients and supplements can interact with medications or potentially affect how cancer treatments work. The evidence is particularly complex for concentrated antioxidant therapies.


For this reason, I take a careful approach to IV therapy during active cancer treatment. Your cancer diagnosis, treatment regimen, medications and supplements, laboratory findings, nutritional status, and overall health all need to be considered before recommending an infusion.


What about IV vitamin C?

IV vitamin C is one of the most researched forms of IV nutrient therapy in oncology, but it is important to distinguish research interest from established cancer treatment.


Studies have investigated IV vitamin C alone and in combination with conventional cancer treatments. Some studies have reported improvements in quality of life or treatment-related side effects, while other studies have not demonstrated meaningful anticancer effects. Overall, the evidence remains an evolving area of research.


IV vitamin C also isn't appropriate for everyone. High-dose IV vitamin C can pose particular risks for people with certain kidney conditions, a history of kidney stones, glucose-6-phosphate dehydrogenase (G6PD) deficiency, or hemochromatosis.


IV vitamin C should therefore be approached as an individualized supportive therapy—not as a replacement for evidence-based cancer treatment.


Is IV therapy safe?

IV therapy can be safe when appropriately prescribed and administered, but it is still a medical procedure and isn't risk-free.


Potential risks can include:

  • Bruising or discomfort at the IV site

  • Inflammation of the vein

  • Infection

  • Allergic or infusion reactions

  • Fluid or electrolyte abnormalities

  • Complications related to a particular nutrient

  • Other risks related to the specific infusion being administered


The risks depend on what is being infused, the dose, the rate of administration, and the individual's health history.


This is why I don't view IV therapy as something that should be approached as a one-size-fits-all wellness treatment.


What happens during an IV therapy appointment?

Before recommending IV therapy, your health history and goals should be reviewed to determine whether an infusion is appropriate.


Depending on the reason for treatment, this may include reviewing:

  • Your medical history

  • Current medications and supplements

  • Recent laboratory testing

  • Kidney and liver function

  • Nutritional status

  • Relevant medical conditions

  • Your cancer treatment, if applicable

  • Your goals for treatment


If IV therapy is appropriate, the selected infusion is administered through a small IV catheter while you are comfortably seated.


The infusion time varies depending on the contents and rate of administration.


After the infusion, you'll receive instructions about what to expect and when to follow up.


How often do you need IV therapy?

There isn't a universal schedule.


Some people may require a single infusion for a specific clinical reason, while others may have a series of treatments when there is an ongoing indication.


I don't recommend scheduling IV therapy simply because a particular number of treatments is marketed as a "protocol."


The frequency should be based on the reason for treatment and your individual response.


If there isn't a clear clinical reason to continue, more IV therapy isn't necessarily better.


What should I know about IV vitamins and supplements?

One of the most important things to understand about IV nutrients is that the route of administration changes the exposure your body receives.


A dose that may be relatively modest when taken orally can produce very different blood concentrations when given intravenously.


This is particularly relevant with high-dose nutrients such as vitamin C.


Some nutrients are also substantially affected by digestion and first-pass metabolism when taken orally. In these situations, intravenous administration can change the amount of the nutrient that reaches the bloodstream and how the body is exposed to it.


It's also important to consider interactions with medications and other supplements. This is especially important in people taking cancer medications, where certain supplements can affect drug metabolism or other aspects of treatment.


For these reasons, I review supplements and medications carefully before recommending IV nutrient therapy.


What IV therapy can't do

There are many claims about IV therapy online, including promises to:

  • "Detox" the body

  • Boost the immune system

  • Cure chronic disease

  • Reverse aging

  • Burn fat

  • Cure cancer

  • Provide instant energy


These claims should be approached critically.


IV therapy is not a magic shortcut to better health, and there isn't evidence that an IV infusion can replace the fundamentals of health such as adequate nutrition, physical activity, sleep, and appropriate medical care.


Likewise, IV therapy should never be presented as a cure for cancer.


The value of IV therapy depends on the specific intervention, the reason it is being used, and the evidence supporting that use.


Who might benefit from IV therapy?

Whether IV therapy is appropriate depends much more on the clinical indication than on a particular diagnosis.


An IV may be considered in situations where:

  • Oral absorption may be impaired

  • Oral supplementation has not been sufficient to address a documented nutrient need

  • A higher dose or blood concentration is desired than can reasonably be achieved through oral supplementation

  • A nutrient's oral bioavailability is significantly affected by digestion or first-pass metabolism

  • Oral intake is difficult or insufficient

  • There is a specific clinical goal for which IV therapy has an appropriate evidence base


On the other hand, IV therapy may not be appropriate if there is no clear indication or if the potential risks outweigh the potential benefits.


That assessment is part of the treatment.


What should you ask before getting IV therapy?

If you're considering IV therapy, I'd encourage you to ask:


Why am I being offered this treatment?

What evidence supports this particular infusion?

Why is IV administration preferable to oral supplementation in my case?

Could it interact with any of my medications, supplements, or treatments?

What are the potential risks?

How will we know whether it's helping?


These are good questions to ask of any healthcare professional—not just when considering IV therapy.


The bottom line

IV therapy can be a useful clinical tool in the right circumstances, but it isn't appropriate or necessary for everyone.


I approach IV therapy as one component of individualized naturopathic care rather than as a routine wellness treatment. The goal is to determine whether there is a clear reason for an IV, whether the potential benefits outweigh the risks, and whether there are simpler or more appropriate alternatives.


For people undergoing cancer treatment, this assessment becomes particularly important. Any IV nutrients or supplements need to be considered alongside the individual's cancer treatment, medications and supplements, nutritional status, and overall health.


Good IV therapy isn't about putting as many nutrients as possible into an IV. It's about determining what you need—and whether an IV is actually the right way to provide it.
